SCHOOL NURSE Medication Administration at School The safe and effective use of medication for the treatment of illness and disability has enabled many children to attend school. There are circumstances when it is necessary to administer medication to a student during school hours. This may enable the student to attend school, improve or maintain their health status and/or improve their potential for learning. Medication that needs to be taken at school needs to be brought and picked up by an adult. A parent needs to fill out the HMES 503 form in order for medication to be given at school. All medication must be kept locked in the Health Center. Medication must be in the original container with a pharmacy label listing the student's name, medication name, dosage and the transcriber's name. Over the counter medication must also be in the original container with the dosage information. No medication from another country can be given at school. Please remember that cough/throat drops, ointments etc. are considered medication. At no time should any medications be sent in baggies, foil or with a student. If you have any questions please call the Health Center at 480-484-3611.
